Our Clinical Teams are skilled and experienced in assisting patients in a wide range of diagnoses while offering continued support throughout the healing process. Our Clinical Team is comprised of a Physician, Nurse Practitioner, Registered Nurses, Licensed Practical Nurses, Registered Dietitian, and Certified Nursing Assistants. In addition to short-term post hospitalization care, our center offers specialty skilled nursing and long-term/ongoing care when a patient’s care needs are no longer able to be met at a lower level of care. Located in the historic Cully neighborhood of NE Portland, Porthaven Healthcare Center specializes in managing a variety of high acuity patients from short-term rehabilitation to long-term care. Our patient-centered facility and dedicated staff take pride in helping each person live the most active and independent lifestyle as possible.
