Heritage Health is a community-centered senior care campus located in the peaceful town of Minonk, just minutes from Bloomington and Peoria. We are Medicare and Medicaid-certified, providing excellent services to area seniors. In 2017, we were one of only two skilled nursing facilities in the nation to receive the Providigm's Embracing Quality Award!

Our shelter care community consists of 12 private suites, each with a private bathroom. Our campus offers RESTORE Therapy, a comprehensive program including speech, occupational, physical therapies to both short-term and full-time residents. Memory care is accessible for individuals experiencing early symptoms of dementia. You can leave your senior in the hands of our trusted respite care team if you're looking for a short-term care option. Our highly-skilled team can also provide wound care, tracheotomy care, electrical stimulation, IV, and total parenteral nutrition (TPN) if needed.

Advocate Eureka Hospital is close by with medical transportation available to provide a convenient means of travel for each resident. Seniors can benefit from our Walk-to-Dine program, designed to enhance the dining experience. Your loved one can attend services at Minonk Baptist Church, located just a half-mile away, socialize with friends over a game of bingo or participate in various community service opportunities. We also partner with dental, podiatry, ophthalmology, audiology, and clinical psychology practitioners to keep our seniors in excellent health. We have an on-site hair salon for residents, and wireless internet is available for guest and visitor use.

At Heritage Health senior living, resident families are care partners, and we encourage them to be involved in their loved one's plan of care. They are always welcome to join us during our fun-filled events and activities. Several of our residents are familiar faces, as some volunteered previously or were family members of other residents before moving in themselves!

We're a senior community that supports our local community and the causes we love! Monday through Friday, we give away Heritage Heart meals to local seniors. We are a partner and sponsor of the Alzheimer's Association Walk to End Alzheimer's, serve as a site for American Red Cross blood drives, and support the Fieldcrest Knights.
